The Collaborative Care Network leans on our collective strengths and trusting relationships to meet the current community needs for optimal well-being.
The Collaborative Care Network (CCN) is essentially to building pilina (relationships), weave together our collective strengths, and center our communities in Hawaiʻi and across the continental U.S. Built upon the foundation of the NHPI 3R, it shaped by current needs and priorities.
Through this new network, we aim to:
- Increase collective capacity by supporting partners’ growth and weaving together strengths.
- Align strategies across sectors to address root causes and advance long-term resilience.
- Uplift community voices through visual storytelling informed by data.
